Discharge Planning

Patients requiring further skilled care following their stay at Deckerville Community Hospital can be matched with community resources including hospice, homecare, long-term care, and Region VII Area Agency on Aging. The patient’s unique needs and qualifications will be matched to the appropriate service. The patient will be given choices from local companies offering the services they need, and a referral will be sent to the company of his/her choice.
